Dosing 

Valcyte features once-daily dosing convenience.

• 900 mg QD (two 450-mg tablets) is the recommended dose of Valcyte for patients with normal renal function4
• Valcyte should be taken with food, beginning within 10 days of transplantation and continuing until 100 days posttransplantation⁴

Dose Modifications for Patients With Renal Impairment⁴

Cr/Cl (mL/min)	Maintenance/Prevention Dose
≥60	900 mg once daily
40-59	450 mg once daily
25-39	450 mg every two days
10-24	450 mg twice weekly

Note: Strict adherence to dose recommendations is essential to avoid overdose.  Valcyte should not be prescribed for patients receiving hemodialysis. For patients on hemodialysis (CrCl < 10mL/min), a dose recommendation cannot be given.
